**AUDIENCE:  **<span style="color: #bf2600">**FACULTY**</span>** | **<span style="color: #bf2600">**STAFF**</span> 

This activity can gather student feedback on specific aspects of the course, as a highly customizable course evaluation, or for many other opinion-gathering purposes. Several question types are available. Optionally, it can anonymize responses as well as save and reuse feedback templates.

Please note that, like Quiz activities, first, you must create the Feedback activity and then, in a separate process, populate the Feedback activity with questions.

## Creating a Feedback Survey

Turn **Edit mode** on in the top-right corner of the main course page.

Find the section where you want to add the **Book** and click **+ Add an activity or resource**.

Click on **Feedback** to open a new Feedback activity.

### General

There are several options available on the page that opens. Enter a name for this Feedback activity in the **Name** text box. If desired, add a description in the **Description** text box.

### Availability

Use the options on this page to limit when students can share feedback. Don’t forget to **Enable** the setting(s).

### Question and submission settings

Choose whether or not to make submissions anonymous.

**Note:** Anonymous submissions can be traced with activity logs, so the submissions are not truly anonymous. You should inform students of this.

### After submission

Decide what happens after students have completed the Feedback survey. Can choose whether or not to show the analysis page, a completion message, and whether to link to a particular location upon completion of the survey.

### Common module settings

Will need to consider in the **Common module settings** is **Availability**, which can also be set on the main course page. If using **Groups** in the course, choose which group(s) the Feedback activity is meant for here.

### Restrict access

Can **Restrict access** to the Feedback activity if like. For example, may want students to complete an activity before they provide feedback about it.

### Completion conditions

**Completion condition** options can be set here.

### Tags

The normal **Tags** options are available.

When finished, click on **Save and display** to move the question creation stage.

### Adding Questions

With the Feedback activity created and saved, it is time to add the questions. To do this, click on the **Edit questions **button. On this page, click the **Add question** drop-down menu to select a question type.

### Standard options

Most of the options that appear when creating questions are self-explanatory, but a few may benefit from additional explanations. These options appear in many question types.

**Label** – This text box is the “name” of the question. While it is not necessary to designate a label to create a question, it *is* necessary to designate a label to base a follow-up question(s) on the response to that question. For this reason, it is good practice to label each question. (More information in the [Follow-up Questions](#follow-up) section below.)

**Required** – If this option is selected, students must answer this question to be allowed to move to the next (or previous) page.

**Position** – This is the numbered position of the question. Questions can be reordered more easily after the entire feedback activity is created, so you can ignore this option on the question creation page.

## Question types

**Add a page break** – Add a break to divide the feedback form into sections. Additionally, page breaks are necessary before and after [follow-up questions.](#follow-up)

**Captcha** – Ignore; this does not work without site-level configuration.

**Information** – This is not a question type. In the “Question” text box, enter instructions for the Feedback activity. The “information type” text box displays the course or department name.

**Label** – A standard label can add text, an image, or a video to provide additional information or to divide questions into sections. (Not to be confused with the *Label* text field within each question, which is a way to designate a name for each question.)

**Longer text answer** – As the name suggests, this is where to request more expansive answers to questions. Can control the length of responses by limiting the width and number of lines in the response text box. 

**Multiple choice** – Within this question type, choose between single answer (radio buttons), multiple answer (check boxes), and single answer (drop-down menu).

**Multiple choice (rated)** – This option allows attaching a numerical value to multiple-choice questions. It can create a Likert scale, which in turn allows for the numerical comparison of responses across course sections and terms.

**Numeric answer** – This question type allows to specify that the answer be a number within a particular range, which should specify in the question. Example: How many classes do you attend in a typical semester? Range: from 0 to 40.

## Follow-up Questions > Macro (anchor)



Can ask questions based on students’ answers to previous questions. For example, if wanting to first ask, “Do you own a car?” If students answer *Yes*, ask them follow-up questions based on the fact that they own a car. If students answer *No*, they skip past the car-related questions.

### Dependence items

Dependence items are the decision point(s) on which the asking of other question(s) depend. In this example, the dependent item is the question, “Do you have a car?” If students answer *Yes*, a follow-up question is asked. When the first question (i.e., the dependence item) was created, it was labelled (named) *Car, yes/no*.

### Creating follow-up question(s): 

The following question will show only if the answer to the *Car yes/no* question above is *Yes*. (The dependency value must match exactly; it is case sensitive.)

### Additional steps to creating follow-up questions:

If using follow-up question(s), must have a page break before the first follow-up question and after the final follow-up question; page breaks between follow-up questions are optional. 

If follow-up question(s) appear at different points of the Feedback activity, the above rules regarding page breaks apply separately to each (set of) follow-up question(s).

# Templates and Exporting Questions

To save the Feedback activity as a template for later reuse, go to **Actions** and select **Save as new template**. Type a name for the template, and then click **Save**. Currently, the template is only available in the course in which it was created.

To use the same Feedback activity in a different course, click **Export questions**. A file will download to the computer. Save the file for later use. 

To import a saved Feedback file, click **Import questions** and drag the previously saved file into the **Choose a file** box (not pictured).

# Analysis and show responses

A variety of options are available under the **Analysis** and **Responses** areas. The **Responses** area can be accessed by clicking the **Responses** tab. The options and information available depend on the content of the Feedback activity. The **Analysis** area is found under the **More** dropdown menu.
